Specimen Required

Draw blood in a grey-top (potassium oxalate/sodium fluoride) tube(s).

Spin down, separate plasma from cells within 15 minutes of draw,

and send 0.5 mL of potassium oxalate/sodium fluoride plasma

frozen in plastic vial.

Note:   Patient's age and sex is required on request form

                  for processing.
