"Renal Biopsy Patient Information Sheet" is required for all orders

and is included in the Renal Biopsy Kit (Supply T231). Collect specimens

according to the instructions "Renal Biopsy-Procedures of Handling

Tissue for Light Microscopy, Immunohistology, and Electron Microscopy"

in Special Instructions.

Note:    If not ordering electronically, please submit the above

                  Information sheet along with a "Pathology/Cytology In Situ

                  Request Form" (Supply T246) with the specimens.
