Specimen Required

Draw a full, yellow-top (ACD) tube; and send ACD whole blood in

the original VACUTAINER. Invert several times to mix blood.

(Green-top [heparin] tube is not acceptable.)

Forward unprocessed whole blood promptly at ambient temperature.

                      (Supply T576) is available. See Special Instructions

                      for a copy of the form.

                  2. If ordering electronically, please complete and submit a

                      "MayoConnect Additional Test Information Form" (Supply

                      T357 or see Special Instructions) with the specimen. If not

                      ordering electronically, please complete and submit a

                      "Coagulation Request Form" (Supply T237) with the

                      specimen.
