Draw blood in a plain, red-top tube(s) or a serum gel tube(s) following
an overnight (12-14 hour) fast. Patient must not consume any alcohol
for 24 hours before the specimen is drawn. Spin down within 1 hour of
draw and send 0.5 mL of serum frozen in plastic vial.
Note: 1. Patient's age is required on request form for processing.
2. Include information regarding treatment, family
history, and tentative diagnosis.
