paraffin. Include 1 H & E slide.

Note:    1. A pathology report and a brief history are required

             for processing.

                  2.  Please complete a "Pathology/Cytology In Situ Request Form"

                  (Supply T246) or a "MayoConnect Additional Test Information

                  Form" (Supply T357) and forward it with the specimen.

                  Requisitions for this procedure cannot be processed unless

                  the information requested is supplied.
