Blocks and hematoxylin-and-eosin stained slides should be submitted.

Additional special stains will be reviewed, if available.

Note:    1. Biopsy site, physician's name, and clinical diagnosis

             are required on request form for processing.

                  2. If ordering electronically, no form is required with the specimen.

                  If not ordering electronically, please complete and submit  a

                  "Dermatopathology/Immunodermatology Request Form"

                  (Supply T060) with the specimen.

 

Pediatric collection instructions same as adult.
