Specimen Required

Draw blood in a plain, red-top tube(s). (Serum gel tube is

not acceptable.) Spin down and send 1 mL of serum frozen in

plastic vial. Serum for a peak level should be drawn 1 hour

after completion of infusion of an intravenous dose or 2 hours

after an intramuscular or oral dose of vancomycin. Serum for a

trough level is usually drawn 30 minutes before dose.
