The registration fee of $397 (for residents, fellows, nurses or medical technologists) or $530 (for physicians or scientists) includes tuition, conference syllabus (accessible online/downloadable), breakfasts, lunches, break refreshments, and Wednesday evening reception. Although it is not Mayo Medical Laboratories Education Department’s policy to limit the number of registrants for a conference, room facilities may necessitate closing of enrollment; therefore, early registration is strongly advised. Please do not finalize hotel or travel arrangements until you receive written notification. Registration deadline is August 15, 2014.

Cancellation Policy

If you cancel your participation in this course, your registration fee, less a $75 administrative fee, will be refunded when written notification is received by Mayo Medical Laboratories Education Department on or before August 15, 2014. No refunds will be made after August 15, 2014.

Mayo Medical Laboratories reserves the right to cancel or postpone any conference due to unforeseen circumstances. In the unlikely event Mayo Medical Laboratories must cancel or postpone this conference, Mayo Medical Laboratories will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/travel/lodging agencies.

Travel

Rochester, Minnesota, is a friendly city that greets thousands of visitors from around the world each year. The city is served by a modern international airport with multiple flights daily via American or Delta Airlines. Access to and from the airport is provided by taxi, shuttle service, and rental car. The airport is located approximately 10 miles from downtown Rochester.

Note to Travelers: Several cities in the United States are named Rochester. When you make airline reservations and check your baggage, be sure that your destination is Rochester, Minnesota (RST), and that your baggage has been properly tagged.

Guest rooms have been reserved for attendees and their guests with special course rates at each of the following downtown Rochester hotels. To receive the special rate, you must make reservations before the room block is filled or before the expiration date of August 19, 2014, whichever comes first. Reservations will be taken following this date based on space and rate availability. Please identify yourself as a participant of the Clinical and Laboratory Update in Thrombosis, Anticoagulation, and Vascular Medicine conference when making a reservation.

Parking

Parking is available in hotel, and city ramps. The cost for parking is not included in the registration fee. A map indicating the location of downtown parking facilities will be available for download upon registration submission.

Conference attendees are cordially invited to the Conference Reception. This reception offers you the perfect opportunity to make connections with existing and new colleagues. Dress is business casual. Preregistration is requested.

Mall of America, the largest fully enclosed retail and family entertainment complex in the United States, is an easy 90-minute drive or shuttle ride from Rochester. Along with 500 retail stores, restaurants, and nightclubs, the mall features Nickelodeon Universe, the world’s largest indoor theme park, and Underwater Adventures® Aquarium, a walk through an aquarium featuring 15,000 fish.

Rochester Direct shuttle service will pick up at all local hotels for multiple trips to the Mall of America. The cost is $39 per person same day round trip. Rochester Shuttle Service offers 12 daily shuttles serving Mayo Clinic and all Rochester Hotels to and from the Minneapolis St. Paul International Airport (MSP) and Mall of America.
