The registration fee of $325 (for residents, fellows, or medical technologists) or $550 (for physicians or scientists) includes tuition, comprehensive syllabus presented on a flash drive, continental breakfasts, lunches, break refreshments, and Wednesday evening reception. Although it is not Mayo Medical Laboratories Education Department’s policy to limit the number of registrants for a conference, room facilities may necessitate closing of enrollment; therefore, early registration is strongly advised. Please do not finalize hotel or travel arrangements until you receive written notification. Registration deadline is August 1, 2013.

Cancellation Policy

If you cancel your participation in this course, your registration fee, less a $75 administrative fee, will be refunded when written notification is received by Mayo Medical Laboratories Education Department on or before July 26, 2013. No refunds will be made after July 26, 2013.

Mayo Medical Laboratories reserves the right to cancel or postpone any conference due to unforeseen circumstances. In the unlikely event Mayo Medical Laboratories must cancel or postpone this conference, Mayo Medical Laboratories will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/travel/lodging agencies.

Travel

Rochester, Minnesota, is a friendly city that greets thousands of visitors from around the world each year. The city is served by a modern international airport with multiple flights daily via American or Delta Airlines. Access to and from the airport is provided by taxi, shuttle service, and rental car. The airport is located approximately 10 miles from The Kahler Grand Hotel that is in downtown Rochester adjacent to Mayo Clinic facilities.

Note to Travelers: Several cities in the United States are named Rochester. When you make airline reservations and check your baggage, be sure that your destination is Rochester, Minnesota (RST), and that your baggage has been properly tagged.

Travel arrangements are the sole responsibility of the individual registrant.

Lodging Accommodations

Guest rooms have been reserved for attendees and their guests with special course rates at each of the following downtown Rochester hotels. To receive the special rate, you must make reservations before the room block is filled or before the expiration date of July 14, 2013, whichever comes first. Reservations will be taken following this date based on space and rate availability. Please identify yourself as a participant of the Bleeding and Thrombosing Diseases Conference when making a reservation.

Mayo Medical Laboratories is not responsible for expenses incurred by an individual who is not confirmed and for whom space is not available at the meeting. Costs incurred by the registrant such as airline or hotel fees or penalties are the responsibility of the registrant.

Parking

Parking is available in hotel, city, and Mayo patient/visitor ramps.  The cost for parking is not included in the registration fee. A map indicating the location of downtown parking facilities will be available for download upon registration submission.

Tours

Tours of selected areas in the Department of Laboratory Medicine and Pathology, and in the Division of Hematology and Internal Medicine, will be available to conference attendees following the adjournment of the conference on Thursday, August 8 (tours begin at 5:45 pm) and Friday, August 9 (tours begin at 12:30 pm). Tours will take approximately 45 minutes and attendees will have the option of signing up for only one per day. Preregistration is required for participation; the number of participants is limited.

Mayo Clinic

Public tours of Mayo Clinic are provided each day of the week beginning at 10:00 am, except for holidays. The tours originate from Judd Auditorium, Subway Level of the Mayo Building, and last approximately 1.5 hours. The tour includes a 20-minute film on the history and operation of Mayo Clinic plus visits to points of interest in the Mayo, Plummer, Hilton, and Gonda buildings. Conference attendees are cordially invited to the Conference Reception and Hemostasis Product Display. This reception offers you the perfect opportunity to make connections with existing and new colleagues as well as view the displays. Dress is business casual. Preregistration is requested.

Mall of America, the largest fully-enclosed retail and family entertainment complex in the United States, is an easy 90-minute drive or shuttle ride from Rochester. Along with 500 retail stores, restaurants, and nightclubs, the mall features Nickelodeon Universe®, the world’s largest indoor theme park, and SEA LIFE® Minnesota Aquarium, a walk through aquarium featuring 15,000 fish.

Rochester Direct shuttle service will pick up at all local hotels for multiple trips to the Mall of America. The cost is $39 per person same day round-trip. Rochester Shuttle Service offers 12 daily shuttles serving Mayo Clinic and all Rochester Hotels to and from the Minneapolis St. Paul International Airport (MSP) and Mall of America.
