Option 2: Use the printed form

1. Print the Request for Supplies form.
2. Complete the client information at the top of the form.
   NOTE: Client account number, account name, contact person and phone number are required.
3. Indicate all supplies needed.
4. Make a copy of the form, if required.
5. Fax request to 507-284-9314.
