What I have shown here is the odds ratio from varying complications from warfarin therapy. In the middle between INR 2 and 3 is the therapeutic range.
Below 2 there is an exponential increase of odds of having a thrombotic stroke. Above 3 there is an increase risk for having cerebrovascular bleeding; so the goal is to maintain the patient as closely as possible between an INR of 2 and 3 in most applications.
With some patients who have older heart valves they might require an INR as going as high as 3.5.
