Significant Warfarin Drug Interactions

Now we also have to take into consideration not just the genotyping but significant drug-drug interactions with warfarin. There are several that should be mentioned because they marked changes in the INR in just a few days. Trimethoprim-Sulfamethoxazole will increase the INR by approximately 50% in 2-3 days if given to a patient, Metronidazole, another -azole antibiotic, will increase the INR greater than 50% in roughly 2- 3 days. Cimetidine increases the INR in almost all patients receiving warfarin and it is recommended that they be switched to other antacids.
